Forrest (Fo) Burke

Founder & Chief Executive Officer

Forrest (Fo) Burke has spent over 30 years leading and building healthcare organizations that deliver value to patients, providers, and payors. Fo began his career as a healthcare attorney working with several of the top delivery systems. He was subsequently hired to serve as UnitedHealthcare’s General Counsel. Fo discovered his entrepreneurial talents at UHC where he led a series of businesses in the commercial and Medicare markets. In these roles, Fo became increasingly focused and committed to finding innovative ways to support patients with complex health needs.

Fo founded PeeqHealth to serve the unique needs of people struggling with behavioral health challenges. PeeqHealth partners with providers to accelerate access to evidence-based behavioral and medical care.

Peeqhealth Mehul Mankad

Mehul Mankad, MD

Chief Medical Officer

Dr. Mehul Mankad brings over 20 years of psychiatric and health-system knowledge from a diverse array of clinical, administrative, and academic enterprises. Immediately prior to his role at PeeqHealth, Dr. Mankad served as Chief Medical Officer at Alliance Health. While there, he led the development of integrated health solutions designed to improve health outcomes in special needs populations.

Dr. Mankad served for 16 years in the Veterans Health Administration including tours as chief of psychiatry and system redesign leader. He maintains a faculty appointment at Duke University where he has received Teacher of the Year from the Department of Psychiatry (on two occasions) and has been published in scientific literature. Dr. Mankad pursued his undergraduate and medical education at Northwestern University. Subsequently he completed his residency and chief residency at Duke University.

As PeeqHealth’s Chief Medical Officer, Dr. Mankad is devoted to excellence in clinical outcomes for the individuals served by network providers. He is passionate about developing and scaling proven clinical models and driving cost-effective care that offers meaningful impact.

Chief Growth Officer

Perry Cannon has spent over 30 years in healthcare working with large global employers and payors. His core goal has been to help consumers navigate the healthcare system and, in the process, deliver value to all stakeholders. During his tenure, Perry led teams across multiple disciplines including sales, operations, and consulting.

As PeeqHealth’s Chief Growth Officer his mission is to help payors, providers, and employers improve access and quality of care in behavioral health and, at the same time, address the cost challenges that trouble our nation’s healthcare system.

Henry Tobie

Chief Development Officer

Henry Tobie brings over 20 years of diverse experience in healthcare and a particular passion for working and collaborating with medical providers. Most recently, Tobie led the recruitment and acquisition team for DuPage Medical Group, now Duly Health and Care, where his team added 150+ providers per year. Before then, he was Vice President of Business Development at ATI Physical Therapy, the largest physical therapy company in the US.

As Chief Development Officer, Henry provides strategic oversight in the areas of business development, physician recruitment, and hospital partnership integration. A passionate executive, he is driven by a desire to make a difference in healthcare and the impact it has on people’s lives.

Peeqhealth John Haben Gs

John Haben

Chief Network Officer

John Haben spent the last 30 years at UnitedHealthcare building and contracting high-performance networks and leading teams in operations and health plan integrations. During that time, he led multiple teams focused on fostering strong provider relationships, reducing medical costs, and incenting value-based outcomes.

John is passionate about helping people navigate the complex healthcare system and partnering with others who share that passion. In 2019, he was appointed by the US Secretary of Transportation to serve as the health insurance industry representative for the DOT’s Air Ambulance and Patient Billing Advisory Committee. He served in that role through its successful completion in the Summer of 2021.

As PeeqHealth’s Chief Network Officer, John’s focus will be to partner with providers and build high-performing networks that accelerate and improve access to care – positively impacting people’s well-being.

Peeqhealth Andrew Mackenzie

Chief Marketing Officer

Andrew has spent over 35 years building marketing organizations that connect with their audiences.

He spent the last 12 years as a Chief Marketing Officer in two of America’s largest health organizations – UnitedHealthcare and Anthem. During that time, he had responsibility for the Medicaid, Medicare and Employer markets. In each of these organizations, Andrew had a passion for finding ways to connect with individuals across the health spectrum, helping them navigate the health system, and live healthier lives.

As PeeqHealth’s Chief Marketing Officer, Andrew is bringing an outside-in approach, making sure that we are listening and understanding those we serve, from Payors to Providers and Patients, and designing a PeeqHealth experience that delivers value.

Peeqhealth Dr Stuart Levine Gs

Dr. Stuart Levine

Chief Medical Officer & CPF Operating Partner

Dr. Levine, a board-certified psychiatrist, has spent his career as a leader in the transformation of clinical care models and delivery system innovation, working to find more effective intersections of primary care and behavioral health.

Dr. Levine was a co-founder of Agilon Health where he helped create a platform that integrates and coordinates care in multiple geographies powered by advanced technology.

Prior to Agilon, Dr. Levine served as the Chief Innovation and Clinical Care Officer at Blue Shield of California where he had a broad range of responsibilities including leading the state-wide California ACO initiative, clinical optimization across all partner provider groups/systems, government programs and clinical innovations for the enterprise.

Among his many other accomplishments, Dr. Levine was also the Corporate Medical Director and Regional Medical Director of HealthCare Partners Medical Group of California where he was responsible for the company’s clinical-focused initiatives for 6 years.
